TRANSCRIPT REQUESTS

 

IN PERSON:  A signed release and a $2.00 fee are needed before any past student transcript can be sent.  If the student is eighteen (18) years or older, they MUST sign the release; guardians are no longer able to sign for the student.  There may be up to a 10-day processing wait.  You must show proof of identification.  All school fees must be paid before transcript requests are processed.

 
BY MAIL:  The student may send a written request to the appropriate school.  The information needed in the request is listed below.  All school fees must be paid before transcript requests are processed.

1.  First Name – Middle Name - Last Name (Married AND Maiden, if there are both)
2.  Date of Birth
3.  Year Your Class Graduated
4.  Address(es) to mail transcripts
5.  Signature of Student
6.  $2.00 fee for every three transcripts (cash, check or money order)
7.  Phone Number

Please contact the District Registrar at the District Office or the counselor on the appropriate campus to fill-out a Transcript Information Release Form.   If the student is eighteen (18) years of age or older, he/she MUST sign the release.
